The second reason is that it hides. Nocturnal hypertension almost never causes symptoms, and a home cuff used during the day will usually miss it entirely. People can carry the pattern for years with a "normal" clinic reading on file, while the damage quietly accumulates. That gap between how harmless it feels and what it is doing is the real danger, and it is also why finding it early matters so much.

How dangerous is nocturnal hypertension, by the numbers

The risk is measured as an increase in the odds of a cardiovascular event over years, not a certainty. Here is what the research shows.

The American Heart Association summarizes it plainly: people with abnormal blood pressure during sleep are more likely to develop cardiovascular disease, especially heart failure, and that association holds even when their daytime blood pressure is in the normal range, per an AHA news release on sleeping blood pressure and heart risk.

A 2020 analysis in the AHA journal Hypertension went further. Why a nighttime number carries more weight

It seems odd that a reading taken while you are asleep would predict your health better than one taken while you are living your life. There are a few reasons it does.

Sleep is a controlled baseline. There is no walking, no stress, no coffee, no full bladder, no conversation skewing the measurement. A high number in those conditions is a cleaner signal of the actual load your cardiovascular system is carrying.
